Your Elder Experts understands aging and the many life changes it brings for older people, their families, and caregivers. We offer the expertise of seasoned Geriatric Care Managers and the compassion of family members.
Whether for a one-time consultation, crisis management, or for ongoing, day-to-day care management, we work with elders, their families, and caregivers to enhance the elders' quality of life, encourage independence, and provide a safe and supportive living environment. Assessments, individualized plans of care and ongoing services are billed at an hourly rate.
We are professionals, trained as social workers, counselors, and advocates, who can navigate senior services, housing options, medical information, and day-to-day living to give older people as much independence, meaning, and quality of life as possible.
The Geriatric Care Management program of JF&CS, now known as Your Elder Experts, was founded in 1999 and has grown into one of Greater Boston’s leading care management practices.
