Older adult with care managerYour Elder Experts is a team of experienced Aging Life Care Managers, most of whom are licensed independent clinical social workers (LICSWs) and Master's level clinicians.

Our staff has a wealth of expertise from previous positions at nursing homes, rehabilitation centers, area Agencies on Aging, and work-life consulting. We have care managers with specialized knowledge of Parkinson's disease and other movement disorders, ALS, and Alzheimer's and other dementias.

Dementia

Caring for a loved one with dementia is often challenging, stressful, and frustrating. Your Elder Experts understands and can untangle the knot of issues around caring for someone with these challenges. Our expert, compassionate, supportive care planning and coordination can make the difference between crisis and coping. We offer coaching and successful strategies for family members, care partners, and clients to minimize disruptive behaviors and defuse crises.

We provide a full suite of care management uniquely crafted to address dementia-related care needs. These services include:

  • In-home clinical assessments and safety evaluations
  • Education and supportive coaching

We also collaborate with JF&CS Alzheimer’s/Related Disorders Family Support. Programs include the Memory Café, a welcoming space for people with memory changes and their care partners, and Balancing Act Support and Education Group, a monthly support and discussion group for adults coping with a parent's dementia.

Parkinson’s Disease

Your Elder Experts has specialized expertise and experience to help people with Parkinson’s disease, their families, and care partners.

Day-to-day management of Parkinson’s disease involves a complex balance of drug interventions, communication with an increasing cast of physicians and specialists, emotional support, and family education. Caring for someone often requires balancing the timing of medications with physician visits, meals, activities, and trying to lead a normal life.

Your Elder Experts draws on resources, networking, and support services to improve the quality of life of people with Parkinson's and their families. We partner with the JF&CS Charlotte & Richard Okonow Parkinson’s Family Support program to provide a wide array of specific supports and services for clients and families. Programs include therapeutic movement and dance, the Tremble Clefs choral group, and therapeutic drumming classes. In addition, JF&CS Okonow Parkinson’s Family Support offers a variety of support groups for care partners and adult children of a parent with Parkinson’s.

ALS

Living with ALS (Amyotrophic Lateral Sclerosis) can be very challenging. Care partners and people with ALS are faced with daily decisions regarding assistance with personal care and acquiring assistive technology and medical equipment. Care Managers at Your Elder Experts are familiar with community resources and available benefits to help the person with ALS and their family and friends. We can suggest options regarding homecare, medical equipment, and home modifications and help advocate within the healthcare system, including being present at medical appointments.
